Welcome to the Health Office!
Dear Parents and Guardians,
As the school nurse, I am here to help facilitate health for all students and staff in the Edmunds Middle School community, to create a healthy learning environment for the most optimal learning experience possible.

In trying to do our best to keep our school community as healthy as possible we'd appreciate if you might keep the following in mind:
1. Students should be fever-free for 24 hours before returning to school.
2. Students should not return to school until 24 hours after last bout of diarrhea or vomit.
3. Students diagnosed with strep throat should not return to school until they have taken antibiotics for 24 hours.
4. Student who have an injury that requires them to miss more than two days of gym class need a doctor's note to be excused from physical activity.

Emergency Information Sheet
Every student must have an emergency information sheet on file in the school. This gives us pertinent information about your childís doctor and health insurance in case of emergency. It also provides us your permission to give your student ibuprofen (Advil or Motrin) or acetaminophen (Tylenol) when they have any headaches or muscle aches at school. Students in this age group get headaches and muscles aches often because of all the physical and hormonal changes taking place within their bodies in middle school.

Required Immunizations
By the 6th grade, students should typically have the following number of immunizations: 3 Hep B, 2 MMR, 2 Varicella, 5 DTaP, and 4 Polio. In 7th grade, students are required to have the Tdap PRIOR to school starting. Please keep this in mind if your child has a doctorís appointment over the summer. If your student does not have the Tdap PRIOR to the start of school the district policy states they will not be allowed to attend school. If students do not have the above mentioned vaccinations, your family must fill out the appropriate exemption paperwork required by the state.

Medication at School
As a reminder anytime your student brings medication to school, our district requires all medications to be in locked cabinet in my office. Prescription medication MUST be brought to school in its original prescription bottle, otherwise I cannot give your student the medication. In addition a parent signature and a physicianís signature are required on our district medication administration form to give the student the prescribed medication.

Please keep in mind that if your child needs to take medication at school, the district policy is clearly stated below:
Medications which must be given during school hours will be administered by the Certified School Nurse or his/her designee. Prescription drugs will only be administered by school personnel on the written order of a physician or other health professional authorized to prescribe. A parent or guardian must complete the Request Form provided by the school or supply the necessary information in written form signed by parent/guardian. Prescription drugs will also require a written order from a physician or prescribing health care provider. The physician or prescribing health care provider must complete the appropriate section of the Request Form or a copy of the prescription or order written on office stationary may be attached to the Request Form. Medication must be brought to school in the original container labeled with the student's name. Medications must be kept under lock and key. First doses of prescription medication will be not given at school. Long term medications must be renewed at the beginning of each school year.

If your student has asthma, an asthma action plan from the doctor's office must be mailed or brought in person to the school every new school year. A doctor signature and a parent signature must accompany the stated plan, and the inhaler itself must be brought to the nurses office with the appropriate paperwork. The Vermont Asthma Program has updated the Vermont Asthma Action Plan. It is available on the Asthma Programís website for printing here.
If your student has any type of allergy, an allergy emergency action plan must mailed or brought in person to school every new school year. A parent signature and a doctor signature must accompany the stated plan, and any necessary medication such as an Epi-pen or Benadryl should be brought to school for the specific student it is intended for.
